Waivers For Pagan Unity Festival

Adult Waiver of Participation May 15th - May 18th, 2008 I do hereby state that I wish to participate in activities sponsored by Pagan Unity Festival Inc. at the Pagan Unity Festival. I understand that Pagan Unity Festival Inc. makes no representations or claims as to the condition or safety of the land, structures or surroundings of Montgomery Bell State Park, whether or not owned, leased, operated or maintained by Pagan Unity Festival Inc. I understand that all activities are voluntary and that I do not have to participate unless I choose to do so. I understand that Pagan Unity Festival Inc. does not provide any insurance coverage for my property. I understand that I am responsible for my safety, my own health care needs, and the protection of my property. In exchange for allowing me to participate in the activities of the weekend, I agree to release from liability, agree to indemnify and hold harmless Pagan Unity Festival Inc. and any Pagan Unity Festival Inc. agent, acting within the scope of their duties, for any injuries that may occur to my person or my property. I understand that the workshops at Pagan Unity Festival will be filmed. By my participation in this festival, I grant permission to Pagan Unity Festival, it’s staff and it’s agents to reproduce any photographs taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium in perpetuity. This release shall be binding on myself, successors in interest, or any person or persons who would attempt to sue on my behalf. I UNDERSTAND THAT THIS IS A LEGAL DOCUMENT. I HAVE READ, UNDERSTAND AND AGREE WITH ALL OF THE STATEMENTS AND TERMS IN THIS DOCUMENT. I UNDERSTAND THAT THIS DOCUMENT IS COMPLETE UNTO ITSELF AND THAT ANY ORAL PROMISES OR REPRESENTATIONS MADE TO ME CONCERNING THIS DOCUMENT AND/OR THE TERMS CONTAINED HEREIN ARE NOT BINDING UPON PAGAN UNITY FESTIVAL INC. OR THE AGENTS OF PAGAN UNITY FESTIVAL INC. I EXECUTE THIS DOCUMENT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ALL THE MEANINGS CONTAINED HEREIN.

I understand that Pagan Unity Festival Inc. makes no representations or claims as to the condition or safety of the land, structures or surroundings of Montgomery Bell State Park whether or not owned, leased, operated or maintained by Pagan Unity Festival Inc. I understand that all activities are voluntary and that my child does not have to participate unless I choose to allow him/her to do so. I understand that by my consent to allow my child to participate I voluntarily accept and assume the risk of injury to my child or damage to my child’s property. I understand that Pagan Unity Festival Inc. does not provide any insurance coverage for my child’s property. I understand that I am responsible for my child’s safety, his/her health care needs and the protection of his/her property. In exchange for allowing my child to participate in the activities of the weekend, I agree to release from liability, agree to indemnify and hold harmless Pagan Unity Festival Inc and any Pagan Unity Festival Inc. agent, acting within the scope of his/her duties for any injuries that may occur to my child or my child’s property. This release shall also certify that I am the parent or legal guardian of this child. I understand that if I am neither, I must present a notarized document giving me permission to bring the herein named child to the festival, signed by the parents. I understand that the workshops at Pagan Unity Festival will be filmed.
